Initial Comments:
This report is the result of a revisit conducted on January 29, 2024, at Oxford Valley Pain and Surgical Center as the result of a previous Medicare recertification survey conducted on October 17-18, 2023 . It was determined the facility was in compliance with the requirements of 42 CFR, Title 42, Part 416 - Conditions for Coverage for Ambulatory Surgical Centers.

This report is the result of an unannounced revisit survey conducted on January 29, 2024, following a State Licensure survey completed on October 17-18, 2023, at Oxford Valley Pain & Surgical Center. It was determined that the facility was in compliance with the requirements of the Pennsylvania Department of Health's Rules and Regulations for Ambulatory Care Facilities, Annex A, Title 28, Part IV, Subparts A and F, Chapters 551-573, November 1999.
